- You need a Type 2 Organization NPI on the application form. A Type 1 Individual NPI will not work.
- ModivCare sets rates by state rate schedule. You cannot negotiate base rates. Focus on getting all eligible add-on fees approved instead.
- Your driver files must be complete before you submit. Each driver needs a license copy, background check, drug test, and CPR card. Incomplete driver files restart the review.
